Online Pizza Sign Up Form (Winter 2018)

Wanna take a break from making lunch? Want your kids to enjoy delicious gourmet pizza from The Drake with a little side of fruit and veggies once a week – while you help raise funds for school activities, supplies and trips?

Pizza lunch happens every Friday (or Thursday, if Friday is a PA day).

Here’s the info you need to have:

DATES:
This session runs February 15 to June 28.

COST:
Cost for the entire session is as follows:
1 slice $75
2 slices $110
3 slices $145
(a five dollar discount applies to each additional child).

PAYMENT:
Hurray! You can now use school cash online to pay!
Once you’re done here, log in to your account. You’ll have to go to each child’s folder and choose the number of slices you ordered for them. When signing up a second (or more) kid, select the sibling option.

Or, you can go the old fashioned way.
Keep track of the cost per child as you’re filling out the form (It’s indicated next to the number of slices). In the end, tally it up (unfortunately, this form does not do the math for you), and indicate how much you need to pay. Then send payment to school with your kid, or drop it off in the designated box/envelope in the office.
Make cheques out to “Hawthorne II School Council” and indicate your child’s name and classroom number in the comment line so we can match payment to order.
If you’re using cash – please put it in a sealed envelope with your child’s name and classroom number on it.

DEADLINE:
To make the transition smooth, we ask that you submit forms and payment by Friday, February 9, 2018.
